Benefit
Description
Spouse Coverage Choice of $10,000, $25,000, $50,000, $75,000 or $100,000. Spouse coverage terminates at age 70.
Dependent Children Coverage $10,000 or $25,000 as selected by you. Dependent children are eligible to be covered from 14 days to 19 years. Dependent children's coverage terminates at age 19 unless enrolled in a school as a full-time student and primarily supported by the employee or disabled prior to age 19.
Accidental Death & Dismemberment An amount equal to that selected by you for your spouse or child.
Terminal Illness (Spouse only; children not applicable) 80% of the selected benefit amount to a maximum of $100,000 may be paid for a terminally ill spouse whose life expectancy is 12 months or less.
Waiver of Premium (only if employee becomes disabled)1 Premiums waived for dependent coverage if the employee becomes totally disabled prior to age 60.  Coverage at age 70 terminates and can be converted to whole life policy.
Guaranteed Issues3

Spouse:  $50,000
Children:  $10,000 and $25,000- all guaranteed issue To qualify for guaranteed issue up to $50,000, the covered person must enroll during the initial sign up period. Any increases in coverage in subsequent enrollment periods will require evidence of insurability.
Portability You may take your dependent's coverage at the CMU group rates if you terminate employment.  It will remain a term life policy with evidence of insurability.  More details available in the Benefits Office.  Spouse must port in order for child to port.

Exclusions

Life Suicide is excluded for the first two years after the insured individual's effective date.
Accidental Death & Dismemberment Your plan does not cover any loss resulting from:
  • intentionally self inflicted Injury, suicide or attempted suicide, whether sane or insane;
  • war or act of war, whether declared or undeclared.
  • Injury sustained while full-time in the armed forces of any country or international authority;
  • Injury sustained while riding on any aircraft except Civil or Public Aircraft, or Military Transport Aircraft;

  • Injury while riding on any aircraft:

  • as a pilot, crew member or student pilot;
  • as a flight instructor or examiner; or
  • if it is owned, operated or leased by or on behalf of the Policyholder, or any employer or organization whose eligible persons are covered under the policy;

  • Injury sustained by voluntarily taking drugs which federal law prohibits dispensing without a prescription, including sedatives, narcotics, barbiturates, amphetamines, or hallucinogens, unless the drug is taken as prescribed or administered by a licensed physician;
  • Injury sustained by operating a motor vehicle while legally intoxicated from the use of alcohol.
