Benefit
Description
Core Life*
Typically is the amount funded by the University's contribution or a minimum of 1 x your current salary
Optional Life*

Basic Accidental Death & Dismemberment*
An amount equal to that selected by you for Core Life.
Optional Accidental Death & Dismemberment
An amount equal to that selected by you for Optional Life.
 
Coma Benefit
If as a result of an accident, an employee becomes comatose within 30 days after an accident, and remains comatose for 31 days after the waiting period, provides 1% of the principal sum for each month a person remains in a coma.  The coma maximum benefit amount equals the principal sum, less all other payments under the AD&D benefit for injury.
 
Conversion Privilege (AD&D)
Allows an employee to convert coverage to an individual AD&D policy without EOI.  Benefit amounts are subject to certain maximums.
 
Critical Burn Benefit
Pays a benefit if the person is critically burned (over at least 25% of his body), which results in a permanent disfigurement as determined by a Physician.  Pays the lesser of 5% of the principal sum; or the actual cost of reconstructive surgery up to a maximum of $5,000.
 
Day Care benefit
If the employee loses his life in a covered accident, this benefit
provides an annual child day care expense payment, for up to 4 years.  The eligible dependent must be under age 11 at the time of death.  The child must be enrolled in a licensed day care program at the time of the accident or will be attending day care within 365 days following the accident.  Benefit is equal to 5% of the principal sum up to a max of $5,000. 
 
Felonious Assault Benefit
Covers violent or criminal acts, involving employer funds that occur while an employee is performing his occupational duties, either in the business premises or while on a business trip.  The benefit is 10% of the principal sum up to a max of $75,000.
 
Paralysis Benefit
The Principal sum is payable for covered accidents sustained by the employee resulting in quadriplegia; 75% of the principal sum for paraplegia; or 50% for hemiplegia.
 
Rehabilitation Benefit
If a covered loss, other than loss of life, provides a benefit for training expenses incurred within 2 years of the accident to help prepare the insured person for an occupation in which he would not have engaged, except for the injury.  Pays 5% of principal sum up to a max of $5,000.
Seat Belt (only if AD&D coverage is paid first)
Provides an additional $10,000 if the employee dies as a result of bodily injuries caused by a covered accident while driving or riding in a private passenger car and was using the car seat belt at the time of the accident.
Air Bag (only if AD&D coverage is paid first)
Provides an additional $5,000 if the employee dies as a result of bodily injuries caused by a covered accident while driving or riding in a private passenger car that was equipped with an air bag, and was using the car seat belt at the time of the accident.
Repatriation
Pays for preparation and transportation of the body for cremation or burial for covered loss of life occurring of the employee outside the state of permanent residence.  Pays 5% of the insured’s AD&D amount subject to a maximum of $5,000.
Spouse AD&D Educational Benefit (only if AD&D coverage is paid first)
Provides for an additional Benefit of the lesser of 5% of Principal Sum or $5,000.  The benefit is payable for a spouse if the optional life is chosen.
Child AD&D Educational Benefit (only if AD&D coverage is paid first)
Provides for an additional Benefit of the lesser of 5% of Principal Sum or $5,000.  The benefit is payable for each dependent who qualifies as a student as the employee has chosen optional life.  Student must be:
·  A full time post high school student on date of employee’s death or a 12th grade student at time of employee’s death and become a full-time post high school student within 1 year.
·  Full-time means registered for not less than 12 course credit hours per semester.
Terminal Illness
80% of the selected benefit amount to a maximum of $500,000 for a terminally ill employee whose life expectancy is 12 months or less.
Waiver of Premium
Premiums waived for the employee if he/she becomes totally disabled prior to age 60.
Guaranteed Issues**
(no proof of insurability)
Employee guaranteed issue amount is 3 x salary (includes base and buy-up) to $575,000 on initial enrollment.  You can go up 1 level each year (except 4X your annual salary) without evidence of insurability.  Core = the amount funded by the University contribution 1, 1.5 or 2 times annual salary depending on employee group (minimum of 1 times salary).  The overall maximum is $750,000.
50% Reduction of Core Coverage
Coverage reduces at age 70 on core only - if hired after 07/01/94, and terminates at retirement or disability retirement.
You may convert your life coverage to an individual whole life policy with no evidence of insurability.  Conversion form and premium must be filed and received by the company within 31 days.  There will be a small set-up policy fee.
An employee may port their coverage when they end their employment, retires, or reduces hours and is no longer eligible for group coverage to a term life policy at the preferred rates.  It can remain a term policy as long as premiums are made.  Portability form and premium must be filed and received by the company within 31 days from date of termination.
Online will preparation at no cost to you.  To access the site you need to go to www.estateguidance.com.  You will also need a code of HFD158 to start the process.  1 will per person. Married couples may complete one will together.  Spouses may complete their own will for a small fee.

Exclusions
Life
Suicide is excluded for the first two years after the insured individual's effective date to any amount that the employee pays all or part of the premium.
Accidental Death & Dismemberment
Your plan does not cover any loss resulting from:
  • intentionally self inflicted Injury, suicide or attempted suicide, whether sane or insane;
  • war or act of war, whether declared or undeclared.
  • Injury sustained while full-time in the armed forces of any country or international authority;
  • Injury sustained while riding on any aircraft except Civil or Public Aircraft, or Military Transport Aircraft;

  • Injury while riding on any aircraft:
  • as a pilot, crew member or student pilot;
  • as a flight instructor or examiner; or
  • if it is owned, operated or leased by or on behalf of the Policyholder, or any employer or organization whose eligible persons are covered under the policy;

  • Injury sustained by voluntarily taking drugs which federal law prohibits dispensing without a prescription, including sedatives, narcotics, barbiturates, amphetamines, or hallucinogens, unless the drug is taken as prescribed or administered by a licensed physician;
  • Injury sustained by operating a motor vehicle while legally intoxicated from the use of alcohol.
